A member of our care coordination team will review the information promptly
We will reach out to the client to initiate next steps and assess fit
The client will be thoughtfully matched with the most appropriate provider
With appropriate consent, we are happy to coordinate care and communicate as needed
Our intention is to ensure the referral process is smooth, respectful, and clinically sound.
